Practice Transformation Specialist ​
Steve Perez has eight years of experience in the health care industry supporting FQHCs. He has notable experience in leading and supporting regulatory and optimization projects, utilizing his knowledge of improving workflows and processes to assist in optimizing health centers’ objectives, assisting in developing new templates and workflows to meet Care Gaps and Health Maintenance Measures. Steve also has experience in working with multi-disciplinary teams teaching and coaching clinical, non-clinical staff and leaders on process improvement methodologies. He holds a Lean Green Belt in Health Care and a Continuous Improvement Professional Award.
As a Practice Transformation Specialist, Steve serves as a practice transformation and process improvement coach supporting assigned health centers and practices on multiple projects. He has experience as a Health Center Controlled Network (HCCN) program coach, supporting his health centers strive towards achieving HRSA defined performance improvement objectives that focus on improving quality of care and patient safety by using health information technology (HIT) to reduce costs and improve care coordination. Steve also supports independent practices and community health centers with improving encounter data knowledge and performance improvement.
Prior to joining the PSO team, Steve held various roles in FQHCs as a Medical Assistant, Medical Scribe and Quality Improvement Specialist. These roles have provided Steve the skills and knowledge to support health centers achieve process improvement from various angles and data reporting sources.
