The one-hour webinar – Elevating Physician Practices: Grant Opportunities for Advancing Population Health and Data Sharing Capabilities – will discuss the data sharing grants from the California Health and Human Services Agency (CalHHS), as well as the California Department of Health Care Services (DHCS) Equity and Practice Transformation Provider Directed Payments program. Data Sharing Grants

With a January 31, 2024, deadline approaching for many entities to begin securely exchanging health information under the Data Exchange Framework (DxF), the CalHHS Center for Data Insights and Innovation (CDII) is offering grants to help physicians comply with the new requirements. California entities that have signed the Data Sharing Agreement (DSA) can apply to receive direct support for DxF implementation, with financial assistance ranging from $15,000 to $50,000, or more via enhanced funding to entities serving underserved communities.

Practice Transformation Grants

The DHCS Equity and Practice Transformation Payments Program will invest in up-stream care models and partnerships to address health and wellness and funding practice transformation.  The program offers up between $375,000 to $10,000,000 for primary care practices that provide primary care pediatrics, family medicine, internal medicine, primary care OB/GYN services, or behavioral health services to Medi-Cal members.
