OFFICE LOCATION: Remote (based in Los Angeles)

EXEMPTION STATUS: Exempt

SALARY RANGE: $85,000-$95,000

POSITION SUMMARY:

The CMA-PSO is looking for a Senior Practice Transformation Specialist (PTS) to work closely with its partner, Aledade, to help facilitate transformative change within independent practices. As a Senior PTS, the ideal candidate will demonstrate advanced technical skills related to providing practice support, quality measure improvement, process-focused work product and strong problem solving experience with respect to direct and indirect client needs. We will also expect strong leadership skills, willingness to lead additional initiatives that may be piloted and/or explored, strong expertise in practice transformation tools and strategies, can navigate conversations with practices regarding improvement needs, and able to execute clear and concise communication and meetings with executive/leadership teams. In addition, the Senior PTS will conduct frequent virtual or (permitting) in-person visits to assigned practices, assisting the physicians and their staff with transformation plans to shift into value-based care, improve quality, and identify opportunities for increased practice efficiency. Finally, the Senior PTS will help CMA-PSO develop its practice transformation process and tools, aimed at building practices’ overall capacity for ongoing and sustainable change to high-performing medical practices.

DUTIES/RESPONSIBILITIES

  • Project manage their assigned practices and work directly with physicians and staff to improve performance related to value-based care, quality improvement and care processes within their practices..
  • Partner with your practices to help design, deploy, and train staff on workflows to incorporate various technologies into their daily activities. The mission is to change the landscape of healthcare delivery for that practice.
  • Regular meetings and presentations with a variety of teams across an organization including clinicians, boards of directors, and executives.
  • Communicate and escalate areas of concern, needed tools, or barriers to achieving goals.
  • Identify barriers and propose solutions for improvement in operations of initiatives and workflows.
  • Assist and guide practices with workflow changes, application and template implementation, clinical calls, and workgroup participation.
  • Lead additional practice initiatives and/or pilots when necessary.
  • Drive the practices toward outcomes that improve care and reduce costs.
  • Gather data from appropriate reports, analyze practice performance metrics on a monthly basis and share with the physicians and practice staff.
  • Assist in the preparation of proposals, reports, updates, and summaries for executive audiences.
  • Other duties as assigned.

QUALIFICATIONS/EXPERIENCE/EDUCATION

  • Five to ten (5-10) years of prior experience working directly with independent practices and/or community health centers (CHCs) across California in quality improvement roles related to practice management, practice transformation, healthcare administration, HEDIS, or healthcare consulting, preferably supporting practices with quality improvement, practice optimization and practice transformation initiatives and transformative workflow changes.
  • Based in California in the greater Los Angeles region is preferable, but would consider general Southern California region. Willing to travel for one onsite visit per practice per quarter. (Note: It is expected that most of the work will be remote, requiring virtual visits).
  • Experience working with multi-site organizations would be ideal.
  • Strong project management experience working on medium to large scale initiatives, preferably in a practice/clinical setting and significant experience presenting and engaging with executive leadership.
  • Experience working on multple projects or tasks requiring input from different teams and leading collaboration efforts with providers, physicians, payors, and other stakeholders to drive consensus.
  • Possess a deep understanding of healthcare systems, payors, value-based care (VBC) and quality improvement methodologies, change management principles, social drivers of health (SDoH), data analytics and regulatory requirements relevant to practice transformation.
  • Experience exporting data and running reports, preparing proposals, reports, updates, and summaries in the practice/clinical setting, process mapping, and workflow analysis tools.
  • Strong understanding of and previous experience working with Electronic Medical Records (EMR) and Practice Management Systems (PMS) for clinical purposes.
  • Experience mentoring junior staff, providing guidance and support to help them develop their skills and expertise in practice transformation.

  • A bachelor's degree in a relevant field such as healthcare administration, public health, business administration, or a related field is preferred, or an AA degree with a minimum of 5 years of PT experience.

EQUIPMENT OPERATION

Position requires operation of normal office equipment, computer, cell phone, printers, photocopier, mobile communications tools, etc.

PHYSICAL REQUIREMENTS

Occasional travel to practices for onsite visits, no more than on a quarterly basis.
